For your initial intake appointment
➤ Log onto eqpsychiatry.com and click on the Patient portal tab
➤ Enter your basic information and choose an appointment slot
➤ You will receive an email and/or text message with a link to a new patient intake form and any other required forms. Once completed online, it will be reviewed within 1 to 3 business days. You will then be notified regarding confirmation of the appointment.
➤ Review the practice policies document for further information regarding appointments.
➤ On the day of your appointment, please ensure you are in a quiet, private space with appropriate videoconferencing software, materials and internet connection.
➤ Prior to your confirmed appointment, you will receive an email and/or text message link.
➤ Please click on this to access your appointment. I recommend logging in 10 minutes prior to your scheduled appointment time in order to work out any technical difficulties.
For follow up/other appointments
➤ Log onto eqpsychiatry.com and click on the Patient portal tab
➤ Click on your appointment type and enter your basic information to choose an appointment slot
➤ You will receive an email and/or text message with a link to any required forms. Within 1 to 3 business days, you will receive information regarding confirmation of the appointment.
➤ Review the practice policies document for further information regarding appointments.

To manage scheduled appointments
➤ Log onto eqpsychiatry.com and click on the Patient portal tab
➤ Click on “Manage appointments” and enter the necessary information to access your scheduled appointments and make any changes necessary.
➤ You will receive an email and/or text message to confirm any changes made.
