Appointments

Scheduling

All scheduling will take place online through the patient portal on https://eqpsychiatry.com

 

When you submit your selected time and date, an intake form will be emailed to you. Once this form is completed and submitted online, please allow 1 to 3 business days for review and confirmation of the appointment.

A credit card is required when booking an appointment. Your card will be charged the FULL amount due for the appointment once your appointment is confirmed.

Regular office hours are Monday to Friday 9am to 6pm. I also offer occasional weekend appointment slots, Saturdays and Sundays between 9am to 5pm.

Cancellation

Once an appointment is scheduled, any cancellations must be made 48 business hours in advance in order to receive a 50% refund.

Cancellations made less than 48 business hours prior to the appointment time will NOT be refunded, and you will be charged the full amount depending on the appointment type.

Rescheduling

Log onto https://eqpsychiatry.com and click on the patient portal. 

Click on manage appointments. 

Enter the email address previously used for appointment bookings. 

Use the code (emailed to you) to access your appointments. 

Rescheduling of appointments must be done at least 48 business hours prior to your appointment, in order to received a 50% refund. 

If an appointment is rescheduled less than 48 business hours prior to the appointment time, it will NOT be refunded. You will be charged the full amount depending on the appointment type. In case of a documented emergency leading to rescheduling, you will receive a full refund. 

No shows

This refers to appointments that are missed without prior notification or cancellation.

No shows will NOT be refunded, and you will be charged the full amount depending on the appointment type.

3 or more consecutive no shows within 1 calendar year will result in termination of the patient-physician relationship.

In order to seek further treatment, the patient is responsible for re-establishing care.

Medications

➤ Due to frequent changes in Federal laws (Ryan Haight Online Pharmacy Consumer Protection Act of 2008) regarding the prescription of controlled substances through telehealth, I do NOT prescribe any controlled substances to patients who are seen only through virtual appointments.

➤ Controlled substances include (but are not limited to) benzodiazepines (such as Xanax, Ativan, Klonopin, Valium etc), hypnotic drugs (such as Ambien, Lunesta etc), stimulants (such as Adderall, Ritalin, Vyvanse etc) and ketamine.

➤ Controlled substances can be prescribed to patients who are able to be seen in person. Regular appointments are essential in such circumstances.
➤ All medications should be taken as prescribed. Never stop or change your medications without first discussing this with me.

Med refills

All medication refill requests must be made by the patient through the patient portal on eqpsychiatry.com
Please do NOT request any refills through your pharmacy.

I recommend all patients to request refills 7 to 10 business days prior to running out of the medication in order to ensure timely refills.

Please allow 2 to 3 business days for refill requests to be processed.
Medication refill requests are only addressed during weekday business hours (9 AM to 6 PM, Monday to Friday).

It is important to follow up in a timely and regular manner to ensure good care. Repeated no-shows or cancellations can result in the denial of medication refills.
For any questions regarding medications or changes in medications, please contact me through the patient portal.

When requesting refills, please provide the following information:

 Your full name and date of birth
➤ Exact medication name
➤ Medication dose (mg) and frequency
➤ Name and phone number of your pharmacy
➤ Your preferred contact number

 

Prior Authorizations

For medications that require prior authorizations, the pharmacy will notify the patient of this at the time the script is filled. It is then the patient’s responsibility to notify the physician to submit a prior authorization. This is to avoid any communication errors that can often occur between pharmacies and physicians offices.
For prior authorizations, please send a message through the patient portal with the following information:
– Exact medication name and dose
– Your insurance carrier
– Insurance group number or member number
– Pharmacy name and phone number

Substance use disorder treatment

I do not provide outpatient treatment of substance use disorders such as detoxification, acute withdrawal or medication assisted treatment.

Paperwork

If you require paperwork (forms, letters) to be filled out, please notify me ahead of your scheduled appointment. This will ensure we have a dedicated appointment or time during your appointment dependent on your needs.

Please note, I do NOT fill out disability, forensic or emotional support animal paperwork.

There is a charge of $50 per document, which will be charged to your credit card on file.
The typical turnaround for paperwork is two weeks.

Emergencies

Equilibrium Psychiatry is NOT a 24/7 emergency provider and messages and emails are not
monitored after office hours.
If you are in crisis and need immediate assistance, please:

For any treatment related communication through the phone in between your appointments, below are the service charges that will be charged to your credit card.
5 minutes or less – no charge
Between 5 to 10 minutes – $15
Between 10 to 20 minutes – $25

Termination of treatment

Reasons for termination of treatment include the following:
– Frequent no shows/cancellations (3 or more consecutively within 1 calendar year)
– Pending/unpaid invoices 90 days after the appointment date
– Non compliance with treatment
– Abusiveness towards staff
– Inability of the physician to provide you with the appropriate level of care

Recording sessions

All appointments (telepsychiatry and in person) can NOT be recorded under any circumstances.

Privacy policies

I am committed to ensure that your health information stays private and secure, as per the Health Insurance Portability and Accountability Act of 1996 (HIPAA).

Billing/payments

Equilibrium Psychiatry operates on a fee for service model, which means that payment by credit card is due at the time an appointment is booked and confirmed.

Any pending or unpaid invoices should be paid within 30 days of the appointment date. No further appointments will be scheduled for patients with unpaid invoices beyond 30 days.

If you have a commercial insurance plan, a “superbill” can be provided which you may submit to your insurance for possible reimbursement. 

Medicare and Medicaid insurance plans are NOT accepted at this time.
